DOT Physical Exam

  • At Rapid DOT, we have a simple 3-step process that keeps things moving efficiently.

    First, when you arrive for your appointment, you'll complete the necessary paperwork (this can also be complete beforehand). Next, you'll be taken into the exam room where you'll meet with our DOT-certified medical examiner.

    The DOT physical exam will include a review of your health history and medications as well as a hands-on medical evaluation. The medical exam portion will evaluate your blood pressure, pulse, vision, hearing, heart, lungs, back/spine, arms/extremities and overall well-being.

    Upon successful completion of the physical, your DOT doctor will sign all the necessary paperwork including your brand new laminated DOT medical card. You information will be updated in the National Registry (note: Lynn CDL holders will still need to complete the self-certification through the MassRMV - more below).

  • A DOT medical card is required for any commercial driver who operates a vehicle with a gross weight of 10,000 pounds or more. This applies to all commercial driver's license (CDL) holders as well as non-CDL holders including hoisting/hydraulic operators, box truck drivers, commercial bus drivers, limousine drivers, and others.

    Drivers of vehicles designed to transport more than 15 people (or 8 people when there is compensation) or hazardous material are also required to maintain a valid DOT medical card.

  • In Lynn (and all of Massachusetts), all DOT exams must be performed by a provider certified through and listed on the National Registry of Certified Medical Examiners. In Lynn, MA, medical doctors (MDs), doctors of osteopathy (DOs), chiropractors (DCs), advanced practice nurses (APN) and physician’s assistants (PAs) are eligible to become “DOT doctors.” All providers at Rapid DOT Physicals are certified to provide “legit and official” DOT exams.

  • To avoid any delay in receiving a new DOT medical card, you'll want to bring a few things to your appointment, including (if applicable):

    >Driver's license (or other form of photo ID)

    >Glasses or contacts

    >Hearing aids

    >A full bladder (for the basic urinalysis)

    >Current medication list

    >Most recent Ha1C (if a diabetic)

    >Form MCSA-5870 if an insulin- dependent diabetic

    >CPAP compliance log (last 90 days, if diagnosed with sleep apnea)

    >Recent tests results and clearance letter from your cardiologist and/or neurologist (if you have a history of heart disease, stroke, seizures or other neurological disorders),

    >Last 12 months of INR readings (if you are prescribed a blood thinner).

  • If you are a CDL holder in Lynn, MA, you are also required to submit a copy of your valid DOT medical card to the MassRMV. This process is commonly referred to the CDL self-certification and can be completed online or via mail.

Our office is conveniently located 6 miles north of downtown Lynn, and is easily accessible via driving or the MBTA system from each neighborhood.

Driving Directions from Lynn:

From West Lynn and Wyoma take MA-107N/Highland Ave to Proctor St (across from the Salem Hospital). Turn left and continue until bearing right onto Pope St. Next, turn left onto Boston St and then a quick right onto Bridge St, then your first right onto Washington St. Church St is your first left, and our office will be the third building on the left.

From East Lynn head north on MA-1A. Take a slight left onto Canal St and continue onto Washington St. Turn right onto Church St, and our office will be the third building on the left.

The Newburyport/Rockport commuter rail stops at Salem Station (outbound from Lynn Station).
