After scheduling your physical either by phone or online you'll receive confirmation of your appointment by email and/or text. In that confirmation, you'll see a link to complete the medical history portion of the paperwork if you choose to.

We also have the necessary paperwork in-office, should you choose to wait.

What To Expect At Your DOT Physical.

When you arrive for your appointment, you'll complete the necessary paperwork (if you haven't already done so beforehand). Then, you'll be taken into the exam room where you'll meet with our National Registry Certified Medical Examiner.

The DOT physical exam will include a review of your health history and medications as well as a hands-on medical evaluation. The medical exam portion will evaluate your vital signs, vision, hearing, heart, lungs, back/spine, arms/extremities and overall well-being.

Upon successful completion of the physical, you will receive your DOT medical card.

Here Are A Few FAQs We Get A Lot

  • Due to the current COVID protocols in Salem, we are unable to accommodate walk-ins. That said, we can guarantee a same-day or next-day appointment. If you need a DOT card ASAP, please call our office and we will squeeze you in.

  • No. The DOT physical does have a urinalysis but it is not a drug test. We will only be checking for sugar, protein or blood in your urine.

  • Yes. The West Church St parking lot with over 100 spaces is immediately adjacent to our building. Parking costs $1.50/hour Monday - Friday 8am - 6pm. Street parking is also close and frequently available.

  • 30 minutes or less. You will not have to wait to be seen.

  • Everyone is required to bring a photo ID (drivers license). If you wear glasses to drive, bring them. Few medical conditions, namely sleep apnea, diabetes, heart procedures and requires additional paperwork as well.

  • In the office we accept cash, card or company checks. We do not accept personal checks. If you choose to prepay online or over the phone for the prepay discount, you can pay be card.

  • Yes, you can. The state through which you have your driver’s license has no bearing on where you are able to get a DOT physical.

  • Yes, we upload your information to the national registry. If you hold a MA CDL, we also offer an optional add-on (+$20 admin fee) to submit your self certification to the MA RMV on your behalf.

Our office is conveniently located 3 miles south of Beverly center and is easily accessible via driving or the MBTA system from each neighborhood.

Driving Directions from Beverly:

From Montserrat, Centerville, Gloucester Crossing, North Beverly, and Downtown Beverly head south on Essex St/MA-22 to MA-1A. Continue straight over the Essex Bridge to Sergeant James Ayube Memorial Drive. Continue onto Bridge St, turning left at the light at Washington St. Take your first left onto Church St. Our office will be the third building on your left.

From the Beverly Farms, Prides Crossing, and Beverly Cove head south on MA-127 to MA-1A. Then, continue straight over the Essex Bridge to Sergeant James Ayube Memorial Drive. Continue onto Bridge St, turning left onto Washington St at the MBTA station crosswalk. Take your first left onto Church St. Our office will be the third building on your left.

From Ryal Side take the Kernwood Bridge to Liberty Hill Ave. Turn left onto MA-114E. Continue straight and follow signs for MA-107N. Turn right onto Bridge St, then your first right onto Washington St. Church St is your first left, and our office will be the third building on the left.
